What Is Wellbutrin (Bupropion)?

Bupropion, sold as Wellbutrin (for depression) and Zyban (for smoking cessation), is a norepinephrine-dopamine reuptake inhibitor (NDRI) — a unique antidepressant mechanism distinct from SSRIs and SNRIs. By blocking the reuptake transporters for both norepinephrine and dopamine, bupropion increases these neurotransmitters in the synapse. Critically, it has no significant effect on serotonin, which explains its key clinical advantages: no sexual dysfunction (unlike SSRIs) and a tendency toward weight loss rather than weight gain. Bupropion also weakly blocks nicotinic acetylcholine receptors, which underlies its efficacy for smoking cessation.

Prescription (Rx) Status

Bupropion is prescription-only. Generic bupropion IR, SR, and XL are widely available. Important contraindication: bupropion lowers the seizure threshold and is contraindicated in patients with seizure disorders, eating disorders (anorexia/bulimia), and those abruptly withdrawing from alcohol or benzodiazepines. An FDA black-box warning exists for suicidal ideation in young patients (as with all antidepressants) and for neuropsychiatric events with smoking cessation use (Zyban).

Uses of Bupropion

FDA-approved: major depressive disorder (MDD), seasonal affective disorder (SAD) (Wellbutrin XL), and smoking cessation (Zyban). Off-label: ADHD in adults (when stimulants are contraindicated), bipolar depression (as adjunct), and weight loss. It is frequently chosen as an antidepressant for patients who experienced sexual dysfunction on SSRIs/SNRIs.

Tablet Strengths

Immediate-release (IR): 75 mg and 100 mg tablets, dosed 3×/day.
Sustained-release (SR, Wellbutrin SR): 100 mg and 150 mg tablets, dosed twice daily.
Extended-release (XL, Wellbutrin XL): 150 mg and 300 mg tablets, dosed once daily. Starting dose: typically 150 mg/day; maximum 450 mg/day (IR/SR) or 450 mg/day (XL). No single dose should exceed 150 mg IR to minimise seizure risk.

Price

Generic bupropion is affordable. A 30-day supply of bupropion XL 150 mg typically costs $15–$40. Wellbutrin XL brand may cost several hundred dollars per month without insurance; generic is the standard.

Frequently Asked Questions

Q: Does Wellbutrin cause weight gain?
A: No — bupropion is associated with modest weight loss (1–2 kg average) and is weight-neutral to weight-reducing, making it one of the few antidepressants that does not cause weight gain.

Q: When is bupropion unsafe?
A: It is contraindicated in bulimia/anorexia, seizure disorders, sudden alcohol/benzo withdrawal, and patients taking MAOIs within 14 days. Discuss your full medical history with your prescriber.

Q: Does Wellbutrin help with ADHD?
A: Off-label, yes — bupropion is sometimes used for ADHD in adults who cannot tolerate or are contraindicated from stimulant medications, though it is less effective than first-line ADHD agents.
