What Is Trileptal (Oxcarbazepine)?
Oxcarbazepine, branded as Trileptal, is a second-generation sodium-channel blocking antiepileptic structurally related to carbamazepine. It is a keto-analogue of carbamazepine and is rapidly converted to its active metabolite (MHD — monohydroxy derivative) which accounts for its antiepileptic action. Oxcarbazepine was developed to improve upon carbamazepine's tolerability and drug-interaction profile. It is a weaker CYP inducer, causes less autoinduction, and has a lower risk of severe haematological reactions (aplastic anaemia). However, hyponatraemia (low blood sodium) is more common with oxcarbazepine than carbamazepine and requires monitoring, especially in older patients and those on diuretics or other sodium-depleting drugs.
Prescription (Rx) Status
Oxcarbazepine is prescription-only. Genetic cross-reactivity with carbamazepine-associated SJS exists, though lower rate — HLA-B*1502 testing recommended in at-risk populations. About 25–30% of patients with hypersensitivity to carbamazepine also react to oxcarbazepine. Monitor serum sodium, especially in first 3 months. Eslicarbazepine acetate (Aptiom) is a further-generation prodrug with once-daily dosing.
Uses of Oxcarbazepine
FDA-approved: monotherapy and adjunct for partial-onset seizures in adults and children ≥2 years. Off-label: bipolar disorder, trigeminal neuralgia, and neuropathic pain.
Tablet Strengths
Tablets: 150 mg, 300 mg, and 600 mg. Oral suspension: 300 mg/5 mL. Starting dose: 300 mg twice daily. Target dose: 600–1,200 mg/day (monotherapy) or up to 2,400 mg/day (adjunct). Titrate in 300 mg/day increments weekly. Lower starting doses and slower titration in renal impairment (CrCl <30 mL/min — use half the usual dose).
Price
Generic oxcarbazepine is affordable (~$20–$50/month). Trileptal brand is no longer manufactured in many markets; generics are widely available.
Frequently Asked Questions
Q: How does Trileptal compare to Tegretol (carbamazepine)?
A: Oxcarbazepine has a more predictable pharmacokinetic profile, fewer CYP-based drug interactions, and lower risk of aplastic anaemia. However, it causes hyponatraemia more frequently. Both are effective for partial seizures; choice depends on individual drug-interaction and tolerability profiles.
Q: Can patients allergic to carbamazepine take oxcarbazepine?
A: Not reliably — approximately 25–30% of patients with carbamazepine hypersensitivity cross-react with oxcarbazepine, including risk of SJS. Careful risk-benefit assessment is required before prescribing.
Q: What are the symptoms of hyponatraemia on Trileptal?
A: Confusion, headache, fatigue, nausea, and in severe cases, seizures or coma. Sodium levels should be checked at baseline and periodically during therapy or if symptoms develop.
